PART I : Shine Team Captain Information

Please complete this section about yourself as the Shine Team Captain. Registrations are grouped by team name and event ID. Please provide a name for your team, which will be printed on your success tools and participant certificates.This form requests basic information to start your Shine Team profile. Additional information will be confirmed when your registration is processed. Please allow 7-14 business days for confirmation.
